What if a recommended practitioner certification is not available for my application?
If you have already visited your practitioner regarding participation in the medical cannabis program and you do not see a recommendation certification available, please contact your practitioner. Your practitioner must first be approved to participate in the program and then submit an electronic certification on your behalf. You will then be able to submit your application. If your practitioner confirms that a certification has been submitted on your behalf, verify the Date of Birth (DOB) and Social Security Number (SSN) used by the practitioner.
